To help us prepare for the August on-campus session, we ask all incoming students to complete this brief Intensive Week Survey. While attendance isn’t required, we encourage all incoming Online and Hybrid MPH students to join the first session in August. It is a great way to connect with faculty, staff, and classmates early in your Dartmouth journey. This survey will help our team best prepare for each student to have the best possible experience and your timely response is appreciated.
Please take a moment to review and complete the required Media Consent Form by June 15.

If you have trouble uploading your photo, please contact the Dartmouth Card office.
To help foster early engagement among your classmates, faculty, and staff, we are creating an incoming class profile to be shared prior to orientation. Please complete the Incoming Student Profile form by June 30.
We recommend using a desktop computer to complete the form, as certain features may not display properly on mobile devices.
Dartmouth Health Services (Dick’s House) requires all students to submit immunization documentation. Please note: both online forms and paper documentation must be submitted by June 30.
Please allow 7–14 days for your forms to be processed. Once processed, you will receive an email from Dick's House to your Dartmouth.edu account confirming compliance or outlining outstanding requirements.
